    Community Health Centers of Oklahoma offer a comprehensive range of services designed to support the well-being of our community. Our offerings include the Women, Infants, and Children (WIC) Program, which provides essential nutrition and support for families alongside personalized Nutrition Counseling to promote healthy lifestyles. We also provide vital Mental Health services, comprehensive dental care, and Family Support Services to assist with various needs.   • Women's Health

    Community Health Centers of Oklahoma provide comprehensive women's health services from adolescence through the senior years, including family planning, midlife health care, prenatal and postpartum care, pregnancy testing, and pap tests. Regular childbirth classes are also available. On-site mammograms ensure convenient access to essential screenings. Additionally, we assist with applications for SoonerCare or SoonerPlan.

  • Children's Health

    Community Health Centers of Oklahoma offer extensive health services for children, including Pediatric and Adolescent Services, Nutrition Counseling, and the Central Oklahoma Healthy Start Initiative. This initiative promotes early entry into prenatal care, provides education on available preventive health services for children from birth to five years of age, and collaborates with the community to prevent unintended pregnancies, focusing on teens. Our dedicated programs ensure comprehensive care and support for children's health and development.

  • Men's Health

    Men’s health services are vitally important because strong individuals equal strong communities. We provide comprehensive services from birth to senior years, including heart health care, mental and emotional well-being support, parenting classes, complete dental services, and a pharmacy. Our offerings also encompass Family Support Services, Health Education, and Lab Services, ensuring holistic support for men’s health at every stage of life.

  • Mental Health

    Community Health Centers of Oklahoma provide vital addiction services, including Medication-assisted Treatment, which helps manage the physical symptoms of opiate dependency, allowing individuals to focus on counseling and recovery. We also offer Screening, Brief Intervention, and Referral to Treatment (SBIRT), an approach for early screening and intervention for substance use disorders and depression. These comprehensive services help individuals gain control of their lives and achieve long-term recovery.

  • On-Site Services

    On-site services at Community Health Centers of Oklahoma include transportation, a pharmacy, mental health services, lab services, and dental care. These comprehensive offerings ensure that our community has convenient access to essential health services all in one location.

    Community Health operates its program and services without regard to race, color, and national origin under Title VI of the Civil Rights Act.

Family Support Services

The friendly staff provides financial, vocational, and educational counseling, mobile meals, and eye exam referrals. Assistance for eligibility and determination for SSI, Social Security, Medical Assistance, Medicare, and Food Stamps is available on-site.

Health Education

School and community group education on various health-related topics can be arranged. Individual counseling and health maintenance are available on-site. Diabetes self-management education classes are held for families and individuals living with diabetes.

Breast Care

Mammograms are available on-site. Community Health can also assist with applying for SoonerCare or SoonerPlan. Visit the Oklahoma Health Care Authority website for eligibility details.

Central Oklahoma Healthy Start Initiative:
Healthy Babies Begin Before Birth

Also known as H4B, it promotes early entry into prenatal care, education regarding available preventive health services for children from birth to five years of age, and collaboration with the community to prevent unintended pregnancy, focusing on teens. The goal is to reduce infant death rates in targeted areas in Oklahoma and Pottawatomie counties.

Women Infants Children (WIC) Program

This program offers nutritious foods and nutrition education to improve your and your child's health. The WIC Program provides food benefits on an e-WIC card to obtain healthy foods that enhance the diets of pregnant or breastfeeding women who have delivered within the last six months and infants and children under five years of age. To qualify, participants must meet income guidelines (185% of the poverty level) and have a brief health assessment by a trained health professional. If you are on Medicaid, Supplemental Nutrition Assistance Program (SNAP)—previously known as Food Stamps or Temporary Assistance for Needy Families (TANF), you qualify for WIC. If you believe you or a family member may be eligible for WIC, please call Mary Mahoney, MHC, at  405-769-1368.

Adolescent Services

Adolescents have special needs, which the Center accommodates with a special registration system. Health promotion and outreach are also available to schools, churches, and civic groups.

Lab Services

Routine blood work, sexually transmitted diseases, HIV testing, urinalysis, and X-rays are performed on-site. Diagnostic X-rays are available by referral to Radiologists.

Transportation

Patients needing transportation must reside within the Center's transportation boundaries and have no other means of getting there.

The Center's transportation boundaries are North NE 122nd Road, South Reno, West Sooner Road, and East Choctaw Road.
